Consent to personal data processing

By submitting a request, creating an account or otherwise providing my data through the livaroq.com website, I freely and on my own behalf give my consent to LLC Livaroq (identification code 445803854, registered address: Georgia, Batumi, Airport Highway Street 186, floor 7, apartment 33; the "Controller") to process my personal data on the following terms.

1. What data

• first name and last name;
• phone number and email address;
• country of residence;
• any information I provide in my request or comment;
• technical data: IP address, cookies, browser details and activity on the website.

2. Health data

Information about my health and my medical records is a special category of personal data. The Controller processes it only if I provide it myself in order to choose a clinic, a doctor or a treatment programme, and only on the basis of my separate explicit written consent, given when I send the documents.

3. Purposes

• contacting me about my request;
• matching a clinic, doctor or treatment programme and arranging consultations;
• arranging travel, accommodation, transfers and payment;
• entering into and performing a contract with me;
• analysing website traffic and improving the website.

4. Processing operations

Collection, recording, organisation, storage, adaptation, retrieval, use, disclosure by transmission, restriction, erasure and destruction — with or without automated means.

5. Sharing, including international transfers

To fulfil my request, the Controller may share my data with clinics, doctors, hotels, carriers and other partners, limited to what is necessary to provide the service. If the chosen clinic or partner is located in another country, I consent to my data being transferred to that country, which may provide a different level of data protection than my country of residence.

6. My rights

I have the right to access my data, have it corrected or deleted, restrict or object to its processing, receive a copy of it, and lodge a complaint with a data protection authority.

7. Duration and withdrawal

This consent remains valid until the purposes of processing are achieved or until I withdraw it. I can withdraw my consent at any time by writing to contact@livaroq.com. Withdrawal does not affect processing carried out before it. After withdrawal, the Controller stops processing and deletes my data within the time limits set by law, unless it has another legal basis to keep it.
